And a small number cannot be taken with Biktarvy at all. The Four Things You Cannot Take With Biktarvy

These are absolute contraindications — not dose adjustments or timing rules. If you are prescribed any of the following, contact your HIV doctor before taking them.

  • Rifampin (Rifadin) — antibiotic used to treat tuberculosis. Lowers bictegravir levels by about 75%, so low that HIV can start replicating again and develop resistance to the medication.
  • Rifapentine (Priftin) — another TB antibiotic with the same effect as rifampin on Biktarvy.
  • Dofetilide (Tikosyn) — used to control heart rhythm. Biktarvy prevents dofetilide from clearing the body normally, allowing it to build up to levels that can cause a dangerous, potentially fatal heart rhythm problem.

Supplements, Antacids, and the Timing Rules

The most common question people have is about supplements and antacids. Certain minerals — aluminum, magnesium, calcium, iron, and zinc — can bind to Biktarvy in your stomach and reduce how much of the drug reaches your bloodstream. How you handle this depends on which mineral is involved.

  • Aluminum or magnesium (antacids like Maalox, Mylanta, Milk of Magnesia; magnesium supplements) — the strongest effect. Take Biktarvy at least 2 hours before, or at least 6 hours after. Taking Biktarvy 2 hours after is not enough.

Statins and Cholesterol Medications

Good news: Biktarvy does not have a clinically significant interaction with statins. Older “boosted” HIV regimens — those containing ritonavir or cobicistat — do require statin dose limits, and this is a common point of confusion. Biktarvy is unboosted and does not meaningfully change statin levels.

  • Atorvastatin, rosuvastatin, pravastatin, pitavastatin: can be used at their standard doses with Biktarvy — no Biktarvy-specific dose cap is required.
  • Your doctor still chooses your statin and its dose based on your cholesterol and heart-disease risk, as they would for anyone.
